Making a referral to the Macmillan Information & Support Service at Tameside Hospital

 

Please note this online referral can be used to refer to The Macmillan information and support services at Tameside Hospital and to Tameside & Derbyshires Macmillan Welfare rights service. One referral covers all three services.   

We want to make it as easy as possible for health and Social care professionals to refer to our service, if you are at all unsure what support is needed, how to access support or whom to refer to Please refer directly to the team using the link below however we do need to know that consent has been given for the referral, preferred method of contact & if we can leave messages, the more information we have concerning clients condition, needs & concerns always helps.  Please complete our online referral form by clicking the green Macmillan information & Support Service Button.

Making a referral to Be Well Tameside smoking cessation service  

 

Be Well Tameside is a holistic, early intervention and prevention service supporting Tameside residents who present a range of lifestyle issues and other factors which impact on their health and wellbeing.

Be Well Tameside offers a free confidential NHS one-to-one Stop Smoking service.  Offering  support, advice and encouragement to help smokers quit smoking for good. Be Well Tameside offers:
 

  • 1-2-1 appointments

  • Regular contact and support to help you achieve your goals

  • Practical tips

  • Information about local groups in your area

  • Help to access other services locally, such as mental health, debt, housing, volunteering and employment.

You are 4 times more likely to successfully quit smoking with specialist support than quitting on your own.

Be Well Tameside is committed to helping our Tameside residents and those who work, stay in hospital or have a GP in Tameside.
Be Well Tameside can support anyone from the age of 16 plus, for stop smoking support there is no age criteria but most pharmacotherapy products can be recommended from age 12 onwards.

Audacious

Making a referral to Audacious Volunteers (Formerly Macmillan Solutions) 

Audacious care solutions offers emotional and practical support to people affected by cancer in their own homes or local community venues. Audacious solutions aim to give beneficiaries more independence, choice and control in their lives, by providing them with a range of support depending on their needs.

Audacious Solutions is available to anyone affected by cancer, whether you have just been diagnosed, are undergoing treatment or are in remission.

Our volunteers support cancer patients, their families, carers and friends.

If you have a need related to cancer talk to us about how we could help you.

Referring to Audacious Solutions is easy. Just pick up the phone and talk to us or send us an email. We accept self referrals as well as referrals from health and social care organisations.

Emotional Support Service at the Tameside Macmillan Unit

In partnership with the Tameside MacMillan Unit, Tameside and Glossop Talking Therapies service offer support and treatment for those who are affected  by cancer and experiencing symptoms such as difficulty sleeping, low mood / depression, stress, worry or anxiety, feelings of hopelessness or panic attacks. The service can also help those dealing with the effects of a long-term health problems or chronic pain, Post Natal Depression, Obsessive Compulsive Disorder, phobias, or disordered eating.

The Tameside and Glossop, primary care talking therapies service is for people over the age of 16 years, who are registered with a GP or reside in the Tameside and Glossop area.

In the referral please indicate clearly that it is for the attention of the Healthy Minds service at the Tameside MacMillan Unit.

​Please be aware that the Tameside and Glossop Healthy Minds Service in is a NON-URGENT Psychological Therapy service for mild to moderate mental health difficulties.

If there is an immediate and /or significant risk, a GP referral to the RAID service is required.

Emotional Support Service for patient who have a Stockport GP

In partnership with the Tameside Macmillan Information and support service, Stockport Talking Therapies service offer support and treatment for those who are affected  by cancer and experiencing symptoms such as difficulty sleeping, low mood / depression, stress, worry or anxiety, feelings of hopelessness or panic attacks. The Srtockport, primary care talking therapies service is for people over the age of 16 years, who are registered with a GP or reside in the Stockport area.

​Please be aware that the Stockport Talking Therapies Service in is a NON-URGENT Psychological Therapy service for mild to moderate mental health difficulties.

Making a referral to Beechwood cancer care  

Beechwood offers support to those living with cancer or a life-limiting illness, as well as their relatives and carers. We first began offering cancer care back in 1990, and since then, our Centre has helped many people.

We understand how cancer and illness can affect someone’s life, and we also know how helpful it can be to talk with other clients and carers.

Visitors to our Centre can choose from a range of support, including:

One-to-one counselling sessions
Complementary therapies
Group sessions with other clients/carers
We can help with every stage of illness, and our Centre vis
itors include those who have just been diagnosed and need a listening ear, people at different stages of treatment and those ready to move on following treatment.

Tameside Live Active Service

 

What is Live Active? 


The Live Active Service is run by Active Tameside and aims to improve health and wellbeing. The programme is designed to work with individuals to help manage their health condition(s) through a tailored physical activity programme, with ongoing support. 

How does the service work? 
Once a referral is received we book the individual in for a consultation to go over health conditions, medication and goals. From there, we advise appropriate sessions, this will consist of a gym program (specific to the client’s current ability, health needs and individual goals) and recommendations to appropriate classes and/or sessions including: 
•    Low level circuit, based on the Cardiac Rehabilitation Phase 4 model. 
•    Strength and Balance following the principles of PSI. 
•    Tai Chi for Falls prevention and Arthritis. 
•    Water Wellness Sessions. 
•    Swimming. 
•    Various community sessions including walk and talk groups, indoor kurling, active allotments and more. 

Who is eligible? 
Anyone with a long-term health condition, who is currently inactive and over the age of 18. 

How long does the scheme last? 
The length of the scheme can vary depending on an individual’s needs, everyone has an initial consultation and a 12-week review, with contact points throughout their time.

ageUK Tameside

Get up and Go

Get up and Go - Age UK Tameside is a support service for older people who want to get back on track, sometimes in life you can feel down, a little stuck, or find it hard coping with major change like loss or bereavement.

Our service offers up to 6 weeks one to one support by qualified professional staff who understand the ups and downs life can bring. Support can be provided in the home or at a local community space close to where you live.

The service also offers group support for those older people who find it helps to be with other people going through similar experiences.

To access the service you need to complete a referral form and return it to getupandgoservice@ageuktameside.com

Please note referrals sent by email from an NHS.net or ​tgh.nhs.uk which are un-encrypted to a .gov.uk email account are not considered secure. To send a secure email use the trust guidance on encrypting and sending emails with secure passwords.  If you do not have access to the secure encryption service or are just unsure what to do, you can send your referral to the Macmillan Information and Support Team who will send your referral securely to Derbyshire`s Macmillan welfare rights officer on your behalf
